Please download the Pre-Registration form from the link above. You may submit the form by choosing from the following options:

• Email. The pdf form is a fill-able form, in which you can type in your information directly in the file.
  • For users who use email programs such as Outlook, Entourage, Mail: To submit the information, click on the link "Email Form". This will launch your email program, attach the pdf and the pdf will be emailed to our office.
  • For users who used web-based email such as Yahoo!, Gmail, HotMail, AOL: To submit the information after you have completed filling out the form, save the file to your computer's desktop or local folder, and manually attach the pdf to your email message to the following email address: dpoirier@tvcca.org.
• Mail. Complete the fill-able pdf form, click on the link "Print Form". Your computer's print menu will open, and you can print to your printer. Please mail to the following address:
