Case Management

TVCCA utilizes the Human Services Infrastructure (HSI) service provision system to holistically assist clients with accessing programs within the agency, within DSS, and throughout the community. At the first visit TVCCA will conduct a comprehensive intake and needs assessment that will provide easy access to TVCCA programs and identify possible eligibility to programs within DSS and other various community organizations. TVCCA will also help to prepare new DSS applications for eligibility determination interviews by explaining DSS programs, assisting with assembling necessary eligibility documentation, and,upon the client's request, by helping them complete the DSS application. TVCCA Case Managers can also assist with referrals to other community resources including but not limited to food banks, furniture banks, and housing authorities.
