This program serves homeless, disabled families and individuals in which the head of household is disabled. Disabilities may include mental illness, a diagnosis of HIV/AIDS, or any other documented disability. Homeless persons throughout New London County are eligible for services.

The Supportive Housing Program (SHP) is designed to provide housing assistance and support services to homeless disabled individuals and families. Support services include comprehensive case management and referral to:

• outpatient healthcare
• mental health services
• substance abuse services
• child care services
• employment and training programs
• landlord/tenant issues and responsibilities
• counseling
• legal services
• state and federal entitlement programs
