Family Bridge Eastern CT Can Help!
The first weeks after birth can be challenging for new families — and new moms especially.
TVCCA’s Family Bridge program is a FREE service for families with new babies in Connecticut to ease their transition from hospital to home. This pilot program—one of two in the state—began on January 1, 2025 and is open for applications.

Family Bridge Eastern CT is a pilot program—one of two in the state. It began on January 1, 2025 and has expanded to accept applications from residents of the following towns:
- Norwich
- Windham
- Griswold
- Lisbon
- Montville
- Mashantucket Tribal Nation
- Mohegan Tribal Nation
- Voluntown
- Salem
- Preston
- Colchester
- Lebanon
- Bozrah
- Sprague
- Franklin
If you live in one of towns listed above and deliver your baby at Backus Hospital in Norwich, Family Bridge Eastern CT can provide you with a no-cost Registered Nurse Home Visit! In addition to a complete physical and behavioral assessment for mom and baby, our RN can answer your questions and assist with access to additional resources in the community.
The program’s Registered Nurses are experts in helping new moms. Your nurse can:
- Give your new baby a check-up
- Give new moms—and family members—a wellness exam, for both physical and emotional health
- Provide education about bathing, feeding, sleeping, safety, and more
Parents can elect to receive up to three visits from the RN, and then receive ongoing support, assistance and referrals to other services if needed from one of TVCCA’s Community Health Workers (CHWs).
TVCCA’s Family Bridge CHWs live and work in the community. They’re experts on local services and resources that help families with a new baby.
They can work with your nurse to:
- Help you get access to diapers and supplies
- Connect you with parenting resources
- Make recommendations for high-quality child care
Whatever you need, our Community Health Workers can help you find it.
Family Bridge adheres to the evidence-based Family Connects model of Universal Nurse Home Visiting.
